Print the form and sign it. Bring it with you to save time at registration. 
Do not write in 'League Use Only' box.

Come in your soccer gear ready to play. Bring your soccer ball and a water bottle. 
You must be accompanied by a Parent or Guardian, and they must sign an AYSO insurance waiver.
